Hospital to Home
When a loved one is admitted to hospital, things can change quickly and discharge can come with little notice. I know this first-hand – my own mum was unexpectedly discharged on Christmas Eve, leaving very little time to arrange everything she needed.
My Hospital to Home service helps take that pressure off you. I can coordinate care, equipment, medication and practical support, liaise with professionals and help prepare the home for a safe return. If your loved one is moving into a care home, I can support the move, help set up their new room and take care of practical arrangements at home – giving you fewer things to worry about at an already difficult time.

As needs change, the home often needs to change too. Reduced mobility, memory difficulties or increasing care needs can make a once-manageable home feel overwhelming or unsafe.
My Home Organisation & Decluttering service can help create a calmer, safer and more manageable space – from reorganising a room or moving a bedroom downstairs to sorting belongings and paperwork, downsizing or preparing for a move. I work sensitively and at your loved one’s pace, respecting the belongings and memories that matter to them while making their home work better for the life they’re living now.

Practical Help & Advocacy
Finding the right support can feel confusing, especially when you’re already managing the practical and emotional demands of caring for an ageing loved one.
My Practical Support & Advocacy service helps take some of that pressure off you. I can help with forms and applications, social care referrals, finding suitable care agencies, liaising with health and social care professionals. I can also attend meetings alongside you, helping you understand your options and ensuring your loved one’s needs, wishes and best interests are heard.
